Every commercial driver needs a current Medical Examiner’s Certificate, renewed at least every 24 months (49 CFR 391.45).
The exam must be done by an examiner listed on the FMCSA National Registry. We book it for you at a clinic near the driver and return the certificate by email.
One price, four steps.

What happens to the certificate.
The examiner submits your result to FMCSA electronically and your state updates your driving record. You no longer need to carry the paper certificate once it shows on your record; keep the copy until then.
Carry the certificate (or a copy) whenever you are on duty, and give your employer a copy for your driver qualification file.
What drivers actually ask.
01How long is a DOT medical certificate valid?
Up to 24 months. The examiner can issue a shorter certificate for conditions that need monitoring, such as high blood pressure or diabetes.
02What happens if I’m not qualified?
The examiner tells you what was found. Any earlier certificate is no longer valid and you must be re-examined and certified before driving a commercial motor vehicle again.
03Do I need a CDL to get a DOT physical?
No. Any driver of a commercial motor vehicle of 10,001 pounds or more in interstate commerce needs one, CDL or not.
04Who pays for the exam?
The rules do not say. Many carriers pay for new hires; many drivers pay for their own recertification. Either can order here.
05What should I bring?
Bring the exam authorization, a photo ID, glasses or contacts, hearing aids, a list of medications, and your current medical card if you have one. Drivers with sleep apnea, diabetes or heart conditions should bring their specialist’s notes or compliance reports.
06Does my employer see the result?
Only if you ask us to send them a copy when you order. They get the outcome and the certificate, never the examiner’s long-form report.
